A multi-disciplinary team at Dartmouth Health developed the Teaching Health Resilience in Inflammatory Bowel Disease (IBD) through Virtual Engagement (THRIVE) clinic in response to the lower-than-expected referral volume of young adults with IBD who were establishing new care at our IBD Center. Accounting for the unique needs of this patient population, we developed a self-sustainable and easily accessible teleclinic to improve engagement in routine IBD care among this high-risk demographic. To date this clinic has been successful in both recruiting new patients and managing follow up care.
