The primary clinical "home" of the Department of Community and Family Medicine is housed at the Community Health Center (CHC), located on Buck Road, about 1 mile from the Dartmouth Hitchcock Medical Center (DHMC). The CHC was established in 1990 in response to a growing demand for primary care services, near the medical center, but in a smaller office-like setting rather than in the large hospital/clinic complex.
The CHC staff includes physicians, physician assistants and nurse practitioners, and houses the clinical practices of 10 Family Physicians, 1 Psychologist, 1 Physician Assistant, and 2 Nurse Practitioners. The CHC's patient services encompass Family Practice, Pediatrics, Sports Medicine, Geriatrics, Psychology, Nutrition Counseling, Physical Therapy, and Laboratory and Radiology.
Primary care at CHC is provided by three care teams composed of physicians, associate providers (nurse practitioners and physician assistants), medical students, nurses, secretaries and other support staff.
