Please submit the items listed below. These items are required in order for the Financial Aid Office to determine your eligibility for federal loans and/or institutional, need-based loans and scholarships. Each of the following items is needed in order to make awards as fair and equitable as possible based upon financial need. It is important that you complete these forms as soon as possible after January 1, 2008 and before April 30, 2008. We will begin processing financial aid applications as soon as we receive the forms. No financial aid award can be made until these forms have been submitted to the Dartmouth Medical School Financial Aid Office.

I. Free Application for Federal Student Aid (FAFSA)

Students applying for federal funds authorized and administered through the U.S. Department of Education should complete the FAFSA application by accessing the web site at:

http://www.fafsa.ed.gov

Please note the following:

1. Be sure to complete the Federal School Code with the following:
   Title IV Code 002573, Dartmouth College, Hanover, NH.
2. No fee is required for the FAFSA.
3. Do not submit this form before January 1, 2008.
4. If you completed FAFSA online for 2007-2008 year, you may complete your renewal FAFSA online for 2008-2009 year instead of beginning a new one.
5. Acknowledgement from FAFSA can be anticipated in the form of a 3-5 page Student Aid Report (SAR).
6. International students are not required to complete the FAFSA, as they are not eligible for federal aid.

II. Need Access Application

Applicants wishing to apply for institutional, need-based loans and scholarships are required to complete the Need Access application through the Access Group's web site:

No one is required to submit this form; however, only applicants who do so will be considered for institutional aid, which includes DMS scholarship and Perkins, DMS, DEA, and DELC loans. If you wish to be considered for institutional need-based funds, parent and spouse year 2007 tax returns and other information (if applicable) must be provided. There is no fee to submit this application for the 2008-2009 year. (Do not submit estimated financial information. If you submit Need Access before taxes are complete, you will be required to submit the application over again.)

III. Dartmouth Medical School Financial Aid Application

Complete and sign the Financial Aid Application then return it to the DMS Financial Aid Office. Please use the space provided to explain any special circumstances which should be considered in the evaluation of your financial need, or attach an additional sheet if necessary.

Federal regulations require that the Default/Refund Verification and the Statement of Need be complete in order to qualify for Title IV funds. These statements are included on side two of the DMS Financial Aid Application. Be sure to sign and date the application on both signature blocks. Loans will not be processed until this application has been submitted to the Financial Aid Office.

IV. 2007 Federal Income Tax Returns

A signed copy of your parents' and a signed copy of your (and your spouse's, if applicable) 2007 income tax return with all schedules and attachments must be submitted to the Financial Aid Office for verification of income submitted on the forms.
